Sample  Submission  Paperwork  Requirements

At Veltec Laboratories, we are constantly striving to meet our customers' needs by providing quality testing services in a timely fashion.

In order to ensure that we are testing to your specific requirements and to expedite your test results, we require all test samples be accompanied by one of our order forms: General Testing order form or Metallographic Analysis order form.  All order forms must include the following information:

• Company Name.
• Sample pick-up location (if applicable).
• Customer contact name and phone number. 
• Purchase order number (if required by your company).
• Method of reporting test results (please provide email address or fax number).
• Type of test(s) required (if mechanical test, indicate what specification you need sample(s) tested to  i.e.  ASTM - JIS - DIN).
• Unique sample identification number(s).  Sample(s) should be identified with an actual number (i.e. heat#, coil#, part#, etc). 
• Please be sure that the identification number on the paperwork is also clearly marked on the steel sample(s) along with the rolling direction.  

We must have the above information before we can proceed with testing.
